Government-Issued Photo ID Driver's license, state ID, or passport for all borrowers on the loan.

Most Recent Mortgage Statement Must show the loan number, servicer name, and total amount past due.

Last 2 Years of Federal Tax Returns (Signed) Include all pages and schedules (e.g., Form 1040).

Last 2 Months of Bank Statements For all checking and savings accounts. Include every page, even blank ones.

Written Hardship Letter A signed letter explaining the hardship, when it happened, and your current situation.

Homeowner's Insurance Declaration Page Proof of active insurance coverage on the property.
